If this is your first visit with us please have all the appropriate paperwork filled out before your visit.
Please reschedule your appointment if you or anyone in your household has been sick in the past 14 days.
Be aware that we hold the right to reschedule your appointment based off your screening of COVID-19.

Special Instructions for Massage Therapy Appointments
Please fill this form out for Massage Therapy and review our Massage Policy before your visit.
Please arrive at least 5 minutes prior to your appointment start time. This will allow you to go through the screening process when entering the office and begin your massage on time. As written in our massage policy, being late to your appointment will result in a time reduction in your massage.
Your massage therapist will be wearing a mask the entire time and the client will be required to have a face covering as well. The massage therapist may choose to wear gloves during your session. You may request for the massage therapist to wear gloves during your session, please make this request prior to the start of your massage.
We are scheduling extra time in between each session to allow for a full sanitation of the massage room for each client. New sheets and blanket will be provided for each new client. Every surface you may come in contact with will be new and sanitized so you can relax and enjoy your session.
If you need to cancel you must give us greater then 24 hours notice. In the event that you cancel with less than 24 hours notice you will be charged a cancellation fee.
